INHOUSE DENTAL PLANS
Adult Membership
Two exams per year.
Two cleanings per year.
All necessary x-rays.
25% on major procedures.
Bitewings and periapical x-rays.
Emergency examinations.
30% on basic dental procedures.
5% on Implant and/or cosmetic services.
Perio Membership
Two regular check-ups per year.
3 periodontal maintenance cleanings per year.
All necessary x-rays, including Full-mouth series (once per 3 years)

Payment Policy
To maintain the valuable fee courtesies included in our membership plan, the following payment policies apply:
- One-year membership must be paid in full at the time of enrollment.
- Membership is valid for one year from the date the annual fee is submitted.
- All treatment fees are due at the time of service.
- For treatments involving multiple visits or lab fees, payment is due at the first visit.
- A $100 deposit is required to reserve appointment time for any specialty service
- Cancellations made without at least 48 hours’ notice may result in a fee and/or forfeiture of the deposit.
- Innovation Dental reserves the right to refuse treatment or terminate membership without notice if the account becomes delinquent.
